Vaccination is one of the most effective and safest preventative measures available to protect yourself and our community from various diseases. The Shiawassee County Health Department (SCHD) offers immunizations for both children and adults. SCHD offers weekly immunizations clinics by appointment, walk-ins are accommodated when possible.

If a parent is not accompanying the child to the immunization appointment, please call in advance to receive consent forms or click the link below to access the Permission Slip for Immunization.

Fees
SCHD participates in the Vaccine for Children (VFC) Program which provides vaccines for free or at low cost. These children must be 18 years and younger and meet one of the following criteria:
- Be enrolled in Medicaid
- Have no medical insurance
- Be an American Indian or Alaskan Native
- Under insured*
*Under Insured: For the purposes of determining eligibility for VFC, children are considered to be under-insured if the child’s insurance does not cover reimbursement for the cost of vaccinations. A child with health insurance that has no immunization coverage at the time the child presents is considered to be under-insured and eligible to receive VFC vaccine.
SCHD also participates in the Michigan Adult Vaccine Program (MI-AVP) for individuals aged 19 and older who have no insurance or who have insurance that does not cover any cost of the vaccine.
SCHD accepts a variety of insurance plans.
Check with your insurance provider to see what costs are covered.
An administration fee of $10.00 is charged with each vaccine for those participating in the VFC and AVP programs, this fee may be waived if there is a financial hardship.
Michigan Care Improvement Registry (MCIR)
The Michigan Care Improvement Registry (MCIR) is a lifespan registry including citizens of all ages in the MCIR that was created in 1998. MCIR benefits healthcare organizations, schools, licensed childcare programs, pharmacies and Michigan’s citizens by consolidating immunization information from multiple providers into one comprehensive immunization record. The MCIR helps healthcare providers view up-to-date patient immunization history in one system.
If you were born before 1998, your immunization record may not have all the vaccines you have received recorded. Historical vaccine entries can be updated in the MCIR system. Individuals interested in having their MCIR updated, must provide proof of vaccination in order to have the MCIR system updated.
Interested in viewing your MCIR or finding out if you are up-to-date on your vaccinations?
- Visit the MCIR Portal to download your State of Michigan immunization record (must be 18 years or older to access)
- Visit or call your healthcare provider
- Visit the Shiawassee County Health Department (must have photo ID to obtain record)
Healthcare providers are required to report all immunizations administered to every child born after December 31, 1993 and less than 20 years of age within 72 hours of administration.
Immunization Waivers
Medical Contraindication Immunization Waiver
Healthcare providers (M.D. or D.O.) can determine an individual’s eligibility for a medical contraindication form. To obtain a medical waiver, please make an appointment with your healthcare provider and have them complete a Medical Contraindication Form. Medical Contraindication Immunization Waivers may not be completed at SCHD.
Non-Medical Immunization Exemption (Waiver)
Completing Online Waiver Education
To obtain a non-medical exemption (waiver), a student’s parent or guardian will need to complete the online waiver education. You can sign up for and complete the waiver education at: https://shiawassee.miwaiv.com/
Please note:
You will need a valid email address to sign up for an account.
There is a verification process that sends an email to you. Please check your junk or spam email box before contacting the Shiawassee County Health Department with log in concerns.
Obtaining the Non-medical Immunization Exemption (Waiver)
The waiver process is not complete until it is signed by the student’s parent or guardian and a Health Department staff member at the Shiawassee County Health Department (149 E. Corunna Ave., Corunna, MI).

Waivers will be signed by Health Department staff during the following walk-in times:
Tuesdays – 9:00am – 12:00pm, 12:30pm-4:30pm
Thursdays -9:00 am-12:00 pm,12:30 pm to 4:30 pm

Frequently Asked Questions
- Where can I get a flu shot? Flu shots are available at SCHD and at many local pharmacies and physician offices. Flu vaccines are typically given between October and June.
- Reporting Adverse Events: Adverse events reported to providers after vaccination should be submitted to the Vaccine Adverse Events Reporting System (VAERS) according to state guidelines. For more information and reporting forms, contact VAERS at (800)-822-7967 or visit the VAERS website.
- My child missed some of their shots, do they need to start over? Short answer, no. The SCHD nurses will be happy to work with each family to determine a vaccine catch up schedule that works for you and your family!
- Is it safe to get so many shots at once? Careful and thorough studies show that getting many vaccines on the same day does not decrease their effectiveness or increase side effects. Additional information can be found here.
- What side effects will my child have after getting their shots? Most side effects are mild and may include a slight fever, fussiness or some redness/soreness at the injection site. Additional information about side effects can be found here.
- How can I prep my child before their immunization appointment? Pack a favorite toy, book, or blanket to provide comfort during the vaccination. If your child is older: be honest about the vaccination process, engage family and friends to support the child, avoid telling scary stories or making threats, and remind the child that vaccines can help protect them from getting sick and keep them healthy.
