About Us: The School Wellness Program (SWP) is a grant-funded, community-based program that includes a nurse and a counselor. These clinicians are committed to helping all High School and Middle School students achieve their best possible physical and mental health. These services are free of charge.
Our Mission: Our Mission is to strengthen students’ physical and mental health by providing therapy and nursing services while at school.
Our Services:
Nursing Services include:
- Physical Assessment
- Care Planning and Case Management
- Nutrition and Health Promotion Education
Therapy Services include:
- Counseling Sessions
- Crisis Intervention
- Referrals and Resources
We are happy to offer services to all middle school and high school students either by appointment or walk-in when available M-F during school hours. After-school and summer appointments will be made available if needed.
