Individuals and businesses can now report COVID-19 cases online! Reporting online will help Shiawassee County Health Department staff streamline case investigations. Please complete the appropriate form below.  Forms are HIPPA compliant. For general COVID-19 questions, residents can call the COVID-19 Hotline at 989-743-2460 monitored Monday - Friday.
